Conditions of Enrolment *

I/We accept as conditions of enrolment that:
1. The herein named student will participate in the general school programme that gives Sacred Heart College its Special Character.
2. We will pay the College Financial Contributions and Attendance Dues. Attendance Dues are approved by the Minister of Education under the terms of the Education and Training Act 2020. Special Character Contributions and Proprietor Contributions collected on behalf of the Proprietor are permitted under the terms of the Education and Training Act 2020. These contributions are eligible for taxation rebate.
3. We will ensure that the policies and rules as laid down by the School Board and the College are observed.
4. Enrolment is subject to the College’s Ministry of Education Enrolment Scheme.

Privacy of Information *

I/We agree to Sacred Heart College collecting personal information on the student. I/We acknowledge that the information provided in relation to the student’s enrolment, and such updated information and other personal information collected from time to time by Sacred Heart College, is collected and used according to the principles of the Privacy Act 2020,  for the purposes of: 
· the Board and/or the College discharging their functions and obligations under the Education and Training Act 2020;
· the administration of Sacred Heart College;
· providing for the student’s educational and general advancement;
· maintaining student records;
· accounting purposes of the Sacred Heart College School Board;
· communication with the Guardians/Parents of students;
· incident reporting and the sharing of information to improve the health and safety and wellbeing of all at Sacred Heart College;
· communication with Sacred Heart College Development Office, Sacred Heart College Old Boys’ Association, Sacred Heart College PTA, NZ Qualifications Authority examination information, Special Education Services,  the Ministry of Social Development (Youth Services) and any other Government agency which may lawfully request such information from time to time; and
· providing to the Proprietor or Proprietor's agent, the Minister of Education and the Education Review Office.
I/We further accept that this information may later be used for statistical and/or research purposes and agree to its use for that purpose, provided that if the information is published in any way it will not identify me/us or the student. 
I/We authorise Sacred Heart College to obtain from any person or organisation any of the student’s personal information required for the purposes of the Board and/or the School discharging their functions and obligations under the Education and Training Act 2020, and authorise that person or organisation to disclose such personal information to Sacred Heart College and/or the School Board of Sacred Heart College.
Unless otherwise indicated, I/we give permission, to Sacred Heart College, that any promotional material for the College involving my/our son is available for College use e.g. website, social media sites, magazines/publications and other promotional material.
The College undertakes to collect, use and store information you provide on this form according to the principles of the Privacy Act 2020.
